Once reservation confirmed, prepare a
Customer Information Record and Boarding Agreement prior to arrival or it can be
completed upon check-in.
On your first visit with us:
A Customer Information and Boarding Agreement
is completed and a review of your vaccination records.
Returning guests/Subsequent visits - if the information provided to Cats Cradle is still
current, you do not need to complete the Boarding Agreement again.
Initials will be required only upon check-in to signify past information provided
to Cats Cradle is still in effect and accurate. You are responsible for supplying
new information if the needs of your cat(s) change and to present
new vaccination records as per expiry dates.
